FINDING 6: The Jail Medical Staff Coordinator failed to submit to the County Risk Manager a Quality Assurance Incident Report as required by Jail Medical Procedure #95-CO68. Health Care Services also failed to conduct a medical review of the in-custody inmate death. This action is in violation of the California Medical Association's "Standards for Health Services in Adult Detention Facilities, Section 115".
RECOMMENDATION 6a: Any time a death occurs in a county jail
facility, the Santa Barbara County Sheriff's Department Major Crime
Investigation Report must include the Quality Assurance Incident Report
prepared by the Medical Staff Coordinator.
RESPONSE - RISK MANAGEMENT: The Risk Management Division
of General Services agrees with the recommendation and will work with
the Sheriff's Department to insure implementation.
